All of the fields in the form are required fields. Please fill out the information completely and submit the service request. The location will be reported to the nearest maintenance facility for evaluation and repair.

These Maintenance Service Requests are handled during business hours only (Monday thru Friday 8am to 4PM). This form should NOT be used to report any kind of highway emergency. Report highway emergencies to 911 or the CHP immediately.
